What is a Reablement Care Plan?
The NHS defines reablement as “a type of care that helps you relearn how to do daily activities, like cooking meals and washing.” Reablement care is often required after a hospital stay, a fall, or an illness that has temporarily impacted your ability to live independently. Once you are discharged from hospital, temporary care may be arranged to support you at home as you recover and readjust to daily life. This care is typically provided for several weeks and will be reassessed to determine when you can manage independently. A reablement care plan will be created for you, involving a multidisciplinary team that may include community nurses, your GP, home carers, and physiotherapists. These professionals work together to ensure you regain your independence as efficiently as possible.
